Below are the most commonly requested forms and documents used at Mission Viejo Christian School. We have divided them into three categories: General Forms, Admission Packet and SCRIP/Fund raising forms. You can simply click the link and view, download and print any of these documents. • Current Newsletter:
  This is the Mission Viejo Christian School current newsletter dated:
  June 1, 2010
• 2010-2011 Handbook:
  The 54 page Student & parent Handbook for the 2010-2011 School Year. Several important changes and updates have been made to the handbook.
• Master School Calendar:
  Two page document showing holidays, minimum days, import events, etc. for the 2010-2011 School Year.
• Elementary Supply List:
  Document explaing the supplies each student will be required to have to be successful and properly prepared for class.
• Junior High Supply List:
  Document explaing the supplies each student will be required to have to be successful and properly prepared for class
• Emergency Medical Form:
  Two page form used for emergency contact information, student allergies, permission to medically treat your child, etc.
• Junior High Bell Schedule:
  Document detailing full day, minimum day schedules along with core subjects and enrichment class scedules.
• Day Care Registration:
  Registration and expination of our Before/After School Day Care Program: "The Crusader Club".
• Dress Code Policy '10-'11:
  This documents explains the authorized uniform styles, colors, etc. Also covered are JH Privileges, Special dress days,etc.
• Student Community Service:
  This four page document explains in detail the Community Service requirements for our students to graduate along with the verification form.
• Extended Absence:
  Mission Viejo Christian School Elementary Extended Absence Contract is needed for students leaving school on scheduled known absences and/or vacations
• Volunteer Driver:
  MVCS is always looking for approved parent drivers to assist in transporting students to & from school events and sports.
• Great American Lunch Box:
  MVCS Elementary uses The Great American Lunch Box to supply our students with HOT Lunches. This form gives menus and website links to order & pay online.
• Lunch Buddies:
  MVCS Junior High uses "Lunch Buddies" to supply our students with HOT Lunches. This form gives menus and website links to order & pay online.
• Conservatory - Fine Arts Classes:
  Drawing & Painting Classes- 9 week sessions, sign up now!.

Admission Application Packet needed in downloadable PDF format

• Application for Admission:
  This is the main Student Application form, use this form along with theParent Form & Statement of Faith.
• Parental Agreement:
  This form requires the signature of the parents and explains the parent's responsibilites.
• Statement of Faith:
  This form explain the core beleifs that make up or Christian Faith here at MVCS and Mission Viejo Christian Church.
• Tuition Fee Schedule 2010-2011:
  This document explains tuition fees, Book Fees and Discounts for additional children.
• Enrollment Form 2010-2011:
  This 2 page document is the enrollment form used for both elementary and junior high school students

SCRIP and Fund Raising

• SCRIP Order Form:
  This two page document has "hundreds" of vendors and prices available for you to chose to meet your SCRIP requirements
• SCRIP Release Form:
  This form authorizes the school to release your SCRIP cards/certificates to your child or designate.
• SCRIP Instructional Booklet:
  This six page booklet describes your responsibilites and the procedures we use with this major fund raising program..
• Raph's Sign-up Information :
  This two page document explains how to register your Ralph's Reward Card and link your purchases to MVCS
• Albertsons Community Partners:
  This document explains how to link your Albertsons Card to MVCS.Once this is done MVCS earns up to 4% of your shopping dollars!
• MVCS Apparel Order Form:
  This form allows you to order Polo shirts, Sweatshirts, Sports Blankets etc.from our SCRIP department.
